National Drug Card - Agent Agreement Summary

By joining the National Drug Card affiliate program, you agree to the following terms:

1. You will receive a unique Group ID number for tracking all claims generated through your distribution efforts.

2. Commissions are paid monthly based on net-paid pharmacy claims processed under your Group ID.

3. Commission rates are tiered: $2.00 per claim (0-500), $2.25 per claim (501-1,500), and $2.50 per claim (1,501+).

4. Sponsorship override of $0.50 per paid claim is earned for any affiliate you recruit into the program.

5. You agree to represent the program accurately and not make misleading claims about the discount card.

6. This is not insurance. The card provides discounts on prescription medications only.

7. All marketing materials, card designs, and content are the property of National Drug Card LLC and may not be reproduced without written consent.

8. There are no monthly fees, charges, or setup costs to participate in the program.
